This Selection Slip is for use by Org Staff to select public to upper orgs.
Instructions
The Selection Slip will be automatically delivered to:
- Selectee *
- Upper Org Terminal for distribution to ASR & FSM I/C
- FCDC Selection to Upper Orgs In-Charge
All sections of the form must be filled in.
* If you don't know the selectee's email address leave it blank in the form. You will be given the opportunity to print the Selection Slip which you should issue to the selectee by personal contact or some other means of correspondence.
